FORM OF A MEDICAL CERTIFICATE.

Lt Col A. J. Alexander, of the 11th Regiment of Ky Vol Cavalry having applied for a certificate on which to ground an application for ~~leave~~of~~absence~~ ^withdrawl of resignation^, I do hereby certify that I have carefully examined this officer, and find that from a personal observation the he is suffring from Internal Hemorrhoids in a severe form, but by prompt treatment it is my opinion that he could ~~[...]~~be made able for field duty in Thirty- or Forty- Days He is under treatment now And that, in consequence thereof, he is, in my opinion, unfit for duty. I further declare my belief that he will not be able to resume his duties in a less period than [ ]
